We promote health through team-based care that involves physicians nursing dietitian pharmacy social work chiropody and home bound services. We provided education/training workshops to both patients and staff. The workshops educated patients on diabetes hypertension anxiety depression or exercise. We continueto conduct research projects related to primary care and share through conferences/workshops and locally.
