London Cares is a housing First initiative aimed at individuals experiencing chronic and persistent homelessness based on a highly collaborative community-based approach. London Cares provides 24/7 street outreach support engaging with individuals experiencing homelessness and supporting them to move off the street. Housing stability workers support individuals that have experienced persistent homelessness who are now housed by assisting them with transitioning into their new home neighborhood and community.
